    ଲଗଇନ ପରେ 'ବିରଥ ରେଜିଷ୍ଟ୍ରେଶନ' ଫର୍ମ ଖୋଲନ୍ତୁ। ଶିଶୁର ବିବରଣୀ, ଜନ୍ମ ସ୍ଥାନ, ପିତାମାତାଙ୍କ ତଥ୍ୟ ଓ ଠିକଣା ସମ୍ପୂର୍ଣ୍ଣ ଭରନ୍ତୁ।After login, open 'Birth Registration' form. Fill child's details, place of birth, parents' information, and address completely.

    ଡାକ୍ତରଖାନା ଡିସଚାର୍ଜ ସ୍ଲିପ ଓ ଆଧାର କାର୍ଡର ସ୍କ୍ୟାନ ଅପଲୋଡ କରନ୍ତୁ। 'ସୁବମିଟ ଦବାନ୍ତୁ। ଆବେଦନ ନମ୍ବର ନୋଟ କରନ୍ତୁ।Upload scans of hospital discharge slip and Aadhaar cards. Click 'Submit'. Note down the application number.
